I heard one theory that Neo and Trinity's "code" remained in the Matrix, where they could remain alive without bodies in the real world. (ie. cached) I wouldn't hate that. In fact, you could run with the theme of whether or not they're still human, or just simulations of humans.
Or, what if they've been turned into "agents" for the machines? (Using their old bodies, and maybe even with Neo's abilities, but maybe as agents for the machines this time.)
In any case, it's Trinity that's really throwing me for a loop. Neo can be fairly easily explained, given that he was the One, became part of The Source, etc. Trinity was just a regular human who physically died in the real world.

The machines can't reproduce consciousnesses, at least not in the first 3 movies. Perhaps since the Merovingian is in this one, maybe he has some way to catch their "soul" or consciousness as it leaves the matrix when they die in the Matrix.
