Curious that docs are recommending this after 12+ days when for over a year I've seen almost nothing but reports that it doesn't work for patients far along at all (and can be dangerous)

Inlaws brought covid for Christmas.

Been sick since the 26th steadily downhill.

O2 fell yesterday to 88 went to urgent care, diagnosiswas mediumcovid pneumonia. They released me after steroid shot. Felt better but last night o2 crashed.

Walking at the house dipped to 76. Cannot catch breath when coughing. Er admitted methey only will do antibiotics, steroids and remdisvir.

Asked the doc and he told me there is nothing else to do said I look terrible for 35 and he would make an exception to try remdisvir even outside the window.

Tough call guys but I literally cannot breathe

I would let the doctors treat what you have to the best of their ability and deal with any repercussions later. At this point, it doesn't sound like you have time on your side to wait it out with only oxygen intervention. Other option would be to have your next of kin start calling other hospitals to see if you can get transferred somewhere with more out of the box experimental care.

Have the doctors mentioned a nebulizer treatment? A friend who was very sick said that when he took his nebulizer treatment it felt like a weight was lifted off his chest and he could breathe again. Not sure which steroid was in the nebulizer and I'm not a doctor. Just relaying a success from a friend.

Hmm.. Remdesivir has generally fallen out of favor as it appears it likely has no significant effect on real world COVID outcomes. Without seeing your medical record my assumption is that the doctors are throwing whatever they can at you in the hopes it may help. Generally speaking I always recommend deferring to the doctor that has eyes on the patient.

I will say my aunt/uncle refused to get the vaccine despite their age - he's now hospitalized after refusing to believe COVID was real. They are trying to give him remdesivir as well, and he was still refusing it. My advice to my aunt (aside from getting vaccinated months ago) was to do what the doctors treating him recommend.

Thoughts and prayers for you Rebel. I received Remdesivir back in September when I was in the hospital with Covid pneumonia. Not for sure if that is what helped, but I am here today fully recovered. If they start this treatment, make sure they are drawing blood twice a day to monitor kidney and liver function. Make sure you are taking Zinc.

What rate of oxygen are you on? And what is you oxygen % at this rate?

Is Tociluzumab (Actemra) still utilized for this scenario? Worth talking to the docs.

This has been used to specifically attack the covid pneumonia response.

Also, Paxlovid is starting to hit shelves.....this may be worth talking about as well

This is a true antiviral, though at this point your problem may be less virus and more immune response......definitely doctor questions but if they were willing to do remd i'd bet they'd use this bullet too

Get better, and if you are feeling down in the hospital just jump over to the football board and read about how much everyone else hates us now, hopefully it will put a smile on your face.

There is a Cochrane review available for this (meta analysis of data). Shows no clear benefit for reducing death, reducing mechanical ventilation. Unclear if it may increase risk of mechanical vent. If I thought I was going to die anyway, I'd take it. If no, I would avoid.

Remdesivir is still being used. It is more effective when given earlier, mainly to pts with significant O2 requirements. Renal insufficiency is a contraindication. I personally have not seen major untoward effects using it on a lot of pts. It is still approved in many hospitals based on the data above. Main benefit is fewer days to recovery.

We are using Baricitinib and tociluzimab as as well. For sicker pts, and more effective if given earlier. Especially pts with elevated inflammatory markers and higher O2 requirements. Main beneftit based on the data is decreased need for mechanical ventilation. If these are given too late - I haven't seen much effect (as well as remdesivir). These 2 are contraindciated with leukopenia.

Dexamethasone is still a cornerstone.

Remdesivir + baricitinib or toci + dex can be given together.

Proning, avoiding the vent still a cornerstone.

I have taken care of too many people so far.

Thanks I was day 10 they made amd exception.

Last night was rough o2 would drop to low 80s and I would have to work an hour to get it to 90, exhausted.

Got my 2nd dose or remdisivir. Not sure if that or the antibodic is working but o2 holding steady at 94.

They mentioned the other treatment but said since I'm improving they will hold off.

Thank you all its insane how tough this is.

OP, if you don't mind sharing, would you consider yourself a pretty healthy 35 year old? Were you vaccinated or have a prior case of COVID?
