This term was previously applied top people during a time when being transvestite was considered a mental illness.
It still is a mental illness, no matter how you you pretend and proclaim it is normal.
Therefore, transvestite is still an applicable term for mentally ill persons that pretend to be the opposite sex by making their appearance look like the opposite sex.

Answering your question though I think there should be a third category. Just like Soviet and eastern bloc athletes, trans women have been under the influence of too much testosterone and it presents an unfair advantage in a women's group. Likewise their hormone levels put them at a significant disadvantage against men. It's the only solution that respects all three groups of people.
