My thoughts on show are:

They keep saying that 48 people survived. Well where are the others, besides who we see each week.

I think the ones on the island are dead maybe even in pergatory. The ones you think died in the crash actually survived.

Anyone else been thinking this scenario?

My guess is that Boone dies tonight... Jack won't be able to save him after all.

Then again, it could be Sayid, or Claire, or Jack, or Kate, or Michael, or Locke, or Jin, or Sun or Sawyer...

awesome...although i wouldn't be suprised to see some kind of "sacrificial lamb" motif where Locke, tired of losing his ability to walk, trades his life to the island for Boone's having taught him everything he knows about hunting and crap. plus, he's assumed a father figure role.

Wasn't my favorite character, but I still would've rather seen Shannon be the one to die. I actually felt a tinge of sadness. This show rocks. wonder what Jack's gonna do to Locke?

In the previews it showed Sayid pointing a gun at Locke. of course in this show I have learned to ignore the previews because they always put a spin on them so things are not as they seem.

I was diappointed... The previews actually gave away too much with Boone. I was hoping that they were going to throw a curve ball and kill off someone else.

What do you think happened to Jack's wife? He doesn't wear a ring and he's never mentioned that he has a wife...

Who here thinks the car wreck had something to do with someone on the island, Locke maybe? Knowing that they all have some little connection to everyone through something, I think this is one of Jack's connections.

Considering how the theme was losing a patient, I thought that some accident befell his wife whom he wasn't able to save, but they never made that point. Something happens because they've never mentioned a wife before.

Who wouldn't want to be married to Julie Bowen? Lucky bowling alley lawyer and lucky Jack.

I'm admittedly a little, perhaps irrationally, disappointed in tonights show. This show is just so darn good I expect a homerun every episode. I really wanted them to shed more insight onto the hull lighting up for Locke in the last episode. Instead we had a set of what looked like triage errors to me.

As soon as Boone's lung collapsed you knew he was gone because Jack was not equiped to repair the internal bleeding/pleural puncture that led to the collapse. His obsessive focus on Boone afterwards while Claire was giving birth was just annoying.

The flashbacks seem to allude to Jack being a trauma surgeon in a major city hospital. As such, at his age he would have lost more than a few hundred patients. By now, he would have to be more practical/less emotional than shown. He should have assisted with the birth rather than continue to fight a losing cause. Better yet it should have dawned on someone to grab Sawyer and Michael and have them carry Claire to the cave..doh!

Anyway, I look forward to next week.

If Jack was a trauma surgeon, I'm sure he did lose a lot of patients. But I'm sure he didn't have a connection to any of his patients the way he is connected to most of the people on the island, including Boone. When you are one of 47 people on an island and you've been through all they've been through and you are THE DOCTOR and you can't save someone, you bet your ass you're gonna react the way Jack did.

I thought this was one of the best hours of the show. It was moving and dramatic and gripping.

You fellows are right. This one touched on something I know a little about so I over-analyzed. This is the most entertaining network show thats been on television for a while so my expectations have gotten a bit unrealistic.

I also wonder about the losing patients thing, but from a different angle than you guys.

The ordeal with Jack's wife insinuates that Jack is an incredible doctor. He has a problem letting go, maybe that comes from being one of the best in his field and only losing a handful of patients.

Never know.

Also, looks like we won't be finding out anything about the hatch next week.

Remember the state Locke was in when he left his "father's" place sans kidney. He was a wreck and driving down the road. I wonder if he caused a car crash between Jack and his new bride. His wife was killed and Locke was paralyzed. Just a theory but I wonder how it will play out.

I thought it very unusual that Jack married one of his patients. As a trauma surgeon he would have limited contact with anyone he worked on after they left the hospital. Also, isn't one of the first things they teach you in med school not to get attached to your patients?

I'm also surprised that someone died before the season finale. But, this trade-off theory is incorrect too. After all, who was added after the marshall died, the lady drowned or random guy #1 was sucked into the engine in episode #1?

No such thing as a trauma doc in a big city who only loses a handful of patients. A big percentage of patients arrive in a state in which only Jesus can save them. Those guys and oncologists just have to get used to it. In my experience there is no more down to earth grounded professional than oncologists. Losing that many patients does an amazing job keeping you humble.

yep. Most of these docs have little to no contact with them after the patients are lucid enough to remember meeting them at all. Once the surgery is over and the patient is on an upward trajectory they have others do the maintenance work. You don't see that much bedside chatter from this group. Then again Jack's wife was hot enough to be an exception to any rule....
