In my field of medicine we have classes on restraining people. The last hospital I was at had multiple restraints per unit daily.
We had them as well. Critical care outside ICU does not typically use sedation. So awake and incredibly sick/injured means lots of restraints.

I talked them into doing my chest tube while I was awake... That was quite an experience.
We always did them on awake patients. Just numb it with lidocaine and go.
My cancer follow ups require a camera be shoved up into my kidneys yearly. I told the doctor that one way or another, I will be unconscious for the procedure. It was up to him how we get there. I get full anesthesia each time. Both doctors I've had tell me that they'd do the same thing.
Otherwise, I'd pass out. I do not like to be poked and prodded. I do not care for needles and have passed out getting blood drawn. It's kind of ironic considering I've had T1 diabetes for 38+ year and have given myself many tens of thousands of injections.

He very well could. But he wouldn't have any leverage, and thus would be likely to make less. Seniors rarely make more money. And he would be a year older.
