Someone earlier mentioned he should get on SOTA. He's beyond that, and it'll require surgery. I know he's limited in exercise ability, but you're right, he can't have a long runway at that size and medical history.

I'd be concerned that he would have all kinds of red flags preventing management/corporate sign-off for full time employment. Insurance, travel accommodations, etc. All in addition of above concerns.

Yeah, I get the sense that there was a lot of comforting him with food when he was a kid going thru having his leg lopped off. Using food as a comfort during trauma at a young age can really mess folks up and lead to morbid obesity. Becomes a life-long unhealthy relationship with food
