You can see that there is a large bushy evergreen tree blocking that notch in the building where the ladder is. This is the side of the building facing away from the rally and law enforcement. Would be extremely easy to climb up here unnoticed. Google Street view actually has coverage from this parking lot from 8 months ago the tree there completely obscures that notch from the ground level

Yes 125-150 yards is a very manageable shot for most people experienced with a rifle.
Plus as I imagine it was this guy's first attempt at a shot with the threat of being under fire while doing so, I could easily see nerves causing a pull at firing point.
I'm a pretty good shot, and take neck shots at my deer for the meat. I've missed more than a few times at 150 yards.
