If it shows there and is required to fill in the form, it is absolutely captured. To achieve some degree of true anonymity, I would recommend them using Qualtrics...while it does capture IP and geo-location it is certainly less identifying. It may be turned off, but I'd want proof of capture was not enabled if I cared to be anon.

The optics of it are pretty bad either way.
