Regarding the 2 guests thing. The attendant should err on the side of caution and in the unlikely event the guest complains about too many place settings you simply say you wanted to make sure he had a full place setting for each course/meal.
I know personally at some high end properties I've received a full place setting and ice water for a soup and another for a sandwich.
