you need a passport, passport card, and enhanced driver's license if you are in one of those states that have them, or a nexus card to get into Canada.
to get back into the US,. you can provide same along with some additional documentation incl original birth certificate
that said. plenty of stories of people trying and getting approval with more/less documentation. so some hit/miss based on the day you go, who you get to review your docs, etc.
