My first week at a company (10 years ago), I had a sales rep bragging to me about winning the bid on a $3 million box sale which consisted of delivery of ~200 line items from ~30 different vendors to 5 customer locations throughout the US. After he turned the order in, my boss comes running out of his office to ask the rep where was the line item for shipping. The reps face turned ghost white and he said "I forgot to charge shipping".

As it turned out, shipping from ~30 vendors to different locations across the US cost more than the profit the company was due for "winning" this bid. Moron was fired and I got a good lesson about checking my work.

I was working for a data/tech company back in the day, getting our systems ready for Y2K, working out bugs, etc. Got fed up with the mundane tediousness of that career. Coworkers and I implemented a program to siphon of fractions of pennies from the company's transactions. It worked TOO well, b/c my idiot buddy who coded the program put a decimal in the wrong place. Instantly, millions of $$$ was transferred to our account. We were doomed. Fortunately, some buffoon in the basement went postal, and burned the building down. Evidence of our scheme disappeared. I'm in construction now.

Video conference. Screen switches to your camera and makes you largest on screen whenever you start speaking. One lady thought she was on mute. Ripped a nasty wet fart and of course she popped up big on the screen and everyone knew what happened. Wow.

You were on a video conf w/ Hillary?

On a lighter note, some rig hands and a crane operator at a company I worked for decided to remake the wrecking ball video swinging naked on the headache weight on the stinger line, with hard hat and boots and other PPE on, then post it online.
I've seen this video. Was told that the participants were relieved of their jobs

Two sales directors were chairing a conference call. They hate each other, and everyone knows it. Director A gives opening to meeting and hands it over to Director B. When Director B starts speaking, he "forgets" to mute Director A on webex. What happens next? Director A, muttering under her breathe, starts saying, "you're a cock sucking *******..you're a cock sucking *******. You suck so ****ing bad" over and over again. NO ONE SAYS ANYTHING. Finally, Director B says, "Hey Director A, we all now know how you feel about me. Can you go ahead and mute your phone?"

Both were laid off a month later.

I pulled into the local credit union drive thru the other day to do some business. After sending me the carrier, the teller inside behind the glass turns to her neighbor and says something, at which point her neighbor looks at me, then says something back to her and they begin giggling. They then proceed to alternately look at me, comment to each other and giggle while I waited. Mind you I'm a normal looking dude - no clown costume, body-builder outfit, top hat or fake nose and glasses. I had no idea what they were talking about. When she finally finished my business and sent me the receipt and said 'thank you', I thought for a second and then replied with, 'thank you too - and , by the way, you may want to get this intercom repaired because I could hear everything you two were saying'.

As I drove away and glanced up both of their mouths were hanging open with an 'oh fudge' look on their faces.

Once I was setting up my out of office reply on a Mac and I checked a button (not realizing what it means) that sent the out of office message to EVERY message that was in by inbox.

So I sent about 3,000 messages all over my organization saying "I will be out of the office..." as if I thought I was so important everyone needed to know where I was. My boss got several hundred of them.

So yes, people will do dumb things. And I know, I need to delete my email.

It involved that same accountant, too, and another one from a different business unit. For those wondering, each was scammed via e-mail into wiring a large amount of money to a European country (in which he have NO operations) because the e-mail had an executive's name on it requesting the wire transfer. Neither one asked for an invoice nor did they seek approval from their direct supervisor in Houston to wire the money.

And on top of that, said executive was placing the entire blame on the Houston office.

I get these e-mails a few times a month. I find it unbelievable that an accountant would pay one of those.

At a prior job, we had a quality manager who was a worthless piece of ***** Everyone knew the department foreman did all of the work for this guy. One month during an all-hands, this manager is assigned to do a presentation on his department. Right off the bat, it's obvious he didn't even spend 10 minutes previewing the presentation. One of the final charts showed a progressive drop in quality metrics over the past year which lined up almost perfectly with his start date. I was standing about 10 feet from the plant manager who looked absolutely pissed off. That dude was fired later that month. I am pretty sure the plant manager chose him as a presenter just to give the guy one more chance to embarrass himself in front of the factory employees.

I worked in a headhunting office where we were all in an open "pit" environment. One Recruiter had been trying to get a hold of a prospect at a financial institution for weeks. She finally got her on the phone and was told firmly but politely there was no interest. The Recruiter slams the phone down saying "f****** c**t!" Her phone immediately rings back. She hadn't seated the phone properly when she slammed the hand set down and was heard by the prospect. The prospect says she is calling the office manager immediately...which she did. The Recruiter started packing a box before the conversation with the manager was even over. When the office manager's door opened and he called the Recruiter's name, she said "I quit!" And walked out. At least she could say she didn't get fired...

Some highly intelligent pencil pusher in the corporate office ordered IV poles that had a very narrow base and made the entire rig top heavy when an IV pump was attached. We had multiple reports of iv poles falling over on patients, and eventually had to pull them out of service.

Those same pencil pushers decided that we used too many adult diapers and reduced the floor stock. Then they decided the super absorbent incontinent pads were too expensive, and replaced them with cheaper flimsy cotton pads that didn't actually absorb any moisture. Now when granny peed in the bed, instead of changing one diaper or pad we often had to change out the entire set of sheets and blankets. The pencil pushers notice we were using more linens, which are expensive to launder, and started limiting our supply of linens. At night, Granny often had to sit in her pee for 30 minutes to an hour while we waited on the linen people to come stock us with more supply. Medicare satisfaction surveys tanked. Pencil pushers still haven't gotten it.

Was working in a furniture store on a slow, rainy day and the manager another salesman and I were running around the store playing dodge ball with a nerf football. I popped up from under a dining room table and my manager threw a dart at me. We had no idea a little old lady had walked in and the ball drilled her in the head knocking her to the floor. She laughed it off and was very understanding.

Had a meeting by video the day after Obama was elected. One of the conference rooms was empty except for 3 ladies who were very excited about the election. The decided they were going to hide in the conference room all day to watch tv news broadcasts. Their conference room monitor had been changed to TV mode, and conference speakers turned down. The camera and Mic were still on. They didn't realize that everyone could see them and hear every word they said. (A lot of "white cracker gon' git what's coming to him now", and similar comments).

They didn't get fired. Should have been fired for being so dumb. They had to manually turn down the conference sound so that they could hear the TV. By changing to TV mode they thought "we can't see them anymore, so they can't see us either".

I knew an employee that left his USB plugged into a group computer. The next person was the victim of incredibly disturbing porn. She had to tell her manager because several other individuals saw it. That dude was fired within a week. Now, he's a director at one of the major financial firms.

On an executive conference call to talk about upcoming new products for development. While waiting for the meeting to start, a man and woman up in Cincy didn't realize their laptop cam was on. They got.it.on.

One of our guys clicked 'record.'

I've unfortunately seen this too, although it was a nurse instead of a doctor. Definitely caught a doc's mistake though
