I find it a confusing rule myself but there is no incentive to call it out until someone gets on base. You still have to have 3 actual batters come to the plate to get 3 outs for the inning. the #4 hitter Bruce was ruled out (via 2 unassisted) in the on-deck circle. And it counts as an official AB for him.
the #3 hitter (Flores) had his double taken away and was considered "skipped." His plate appearance was not an official AB.
would be interesting if this scenario came up during a no-hitter or perfect game.
also...not sure why the Reds didn't call out the problem after the #2 hitter (Cabrera) batted as he was penciled into the 3rd spot. However technically I suppose teams can skip batters in their lineup as long as they're willing to take the out too
