100% Agree. The US is actually one of the few countries that allows direct to consumer marketing of drugs. The medical system has turned into Burger King, everyone gets to have it their way. If you don't get a drug from your doctor because you saw a commercial and think you need it, just go to another doctor that is morally corrupt and he'll take your money and prescribe what you want.

Totally agree with you and your statement of people seemingly not able to deal with even minor stress. Seems people claim to have PTSD over the least little thing. While I suppose in some cases it is true, I get the sneaking suspicion that more than a few "have" it to be able to not work, draw disability, etc. I have personally known three people who pulled it off.

You have connected two of the dots, but there is a third piece of the puzzle - the politicians (mostly democrats).

Politicians protect pharma (i.e. no indemnity for Covid), pave the way for huge profits, and grant huge contracts through gov't channels, VA, etc. -->

Pharma contibutes to super PACs and gives ridiculous amounts of money to the media through "advertising" (while they're at it, pushing a social narrative - homosexuality is the norm, obese is beautiful, etc) -->

Media stays silent about drug complications and gives favorable coverage to politicians (again, mostly Dems) -->
And the cycle continues.

This is not unique to pharma, just the most obvious. It's also why a politician with no interest in personal financial gain is so dangerous to the system and gets hammered from every angle.

It is pretty interesting why you see all these commercials. Back when I was in training (1999-2002) we had drug reps in all the time bringing cookies, having presentations, free pens, etc. In addition they would give trips (with education) and even sports tickets to docs to encourage them to rx their drug.

Right when I finished training, in 2002, the wise rulers up in DC decided that pharma was having too much influence on the docs. So they said they can only bring little lunches, no spouses at dinners, no trips, no sports tickets, no pens, no notepads, no coffee mugs BUT they can advertise direct to consumers. Supposedly they thought this would cut pharma costs and result in drug cost decreases.

Instead you get what we have today- direct advertising to people with no medical training. It is a mess and I missed out on free goodies dangit!

The US has always had anti-kickback laws, but drug companies found ways around it, ie lunches, sponsorships to conferences, etc. When the opioid crisis hit the gov't 'was forced' to crack down on all these loophole anti-kickback schemes and this made it harder to get Dr. Smith to prescribe x-drug from Merck or y-drug from Pfizer. So big pharma took a new route and that was to force the hand of the doctor by getting the patient involved in their own prescription requests, which is why every commercial is a drug company with the 'ask your doctor' catchphrase in it. I can guarantee drug companies are still doing kickbacks, however this is the easiest way to get the general public off their backs by giving the perception they can request what the doctor will prescribe rather than thinking the doctor is getting something from the drug company for prescribing it.

I am literally eating dinner and have to be subjected to endless commercials discussing torn colons and severe abdominal pain with chance of death

Came to the thread to point this out, too. Pharmaceutical companies used to spend their marketing budgets on direct marketing to physicians. That included everything from cheap pens and PostIt notes emblazoned with the drug name to hosting CME credit seminars at swanky resorts. Remember when hot, good looking drug reps used to visit doctor offices and bring lots of merch along with cookies and catered lunches? Then, because this was deemed to be terrible, it was outlawed. Well, as usually happens, unintended consequences tend to be worse. Pharmaceutical companies have to market to someone, and if there are heavy restrictions on marketing directly to healthcare providers, then the result is marketing direct to patients. It's not so much a conspiracy by Big Pharma as it is getting around poor legislative attempts to decrease the cost of care. And it backfired. (It always does.)

Exactly this. I remember very clearly moderating an ethics class symposium at LSU med school back then when the discussion was about whether it was appropriate for physicians to accept all-expenses paid "educational" seminars from pharmaceutical companies. Not a single med student thought there was anything wrong with it. "I earned that trip!" was a common justification.

One can argue the non-importation laws mean that Pharma can charge different prices in different countries. Bound to contribute to higher prices in the US.

As an aside, I've seen this issue mentioned as an example how important it is to control what things can be voted on. It is hard to justify $1/pill in Mexico and $100/pill ten miles away in South Texas. But if there is never serious legislation around the issue then you don't have to justify it.
