I think it's a lack of situational awareness. Benn has a problem with this too. Don't take penalties that far away from your own net, especially when it's 1 on 2.

One of our guys used the door inside the blue line to go to the bench instead of hopping over the boards in the middle third, so he was inside the zone when the puck entered.

They're too cautious (but that could be understandable I suppose).
