Army Risk Assessment Card from the 1990s - source?

At work, I'm trying to explain to a bunch of Silicon Valley type computer guys the concept of simple, routine, complex tasks being performed by a team that is trained, untrained, partially trained, etc.

The visual aid that I think would help with this is the old Army Risk Assessment Card that we used in the 1990s.

Obviously I don't need the part about the weather conditions or if the movement is using wheeled or tracked vehicles but from memory, there was a good classification of several various risks on that card.

I've searched the Internet and I can't find an example of that card. And of course I have no idea what the document number was (probably a GTA of some sort).

I'm sure if I dig through some old boxes in the garage I could find an example but if anybody knows where I could find a copy online, I'd appreciate it.

Did it chance much from the 90's to '05?

I'm looking for the one that maybe had a scorecard of about 25 points or something and took into account things like weather, complexity of the tasks, movement by vehicles, etc.

The wording I'm really looking for is about the difficulty of the tasks.
I remember one of the descriptive words was "Routine".

Is that the one you have?

According to this document from 1998, it looks like they probably phased out the generic risk assessment card by the late 1990s in favor of risk assessment that was focused on the unit or the activity.

No, the one I have just lists initial risk level (low, mod, high, extremely high), develop controls, and then residual risk levels, how to implement controls, and who will supervise.
