I wouldn't say more rigorous but have gone from 45 minute rides to 1 hour rides. I think what has confused me was feeling completely fine then waking up and feeling it immediately on my first step and also the fact that with no weight on it I can move the foot in full range of motion and feel no pain whatsoever.

If it is what you are thinking is there a brace or anything to try or is it just going to be rest until it's gone?

Make sure your shoes are laced up well when you are riding. Are you clipped in or not?

I can tell you from my personal experience, Nike Metcons made my plantar fasciitis flare up like a mother. Every time I would try wearing them, it would flare up.
Changed over to Reebok Nanos and haven't had any issues since.

Dear PT - for the last few months, when I run on the treadmill (no incline, 6-7.5 mph) after a little warmup (3-5 mins) and some light stretching, I have strained the same medial head of my gastrocnemius of my right calf now 3 times. I am fine, running along great and then out of nowhere it feels like I was stabbed with an icepick and I have to stop immediately. Never had this happen before.

First time, laid out 2 weeks, second time 3 weeks, now I have been inactive for 2 weeks. RICE for 2 days, occasional anti-inflammatory in the first 48. It doesn't hurt at all after 48 hours. First 12 is fairly uncomfortable, next 36 are lingering pain when going up and down stairs. I use a thera-gun-type device on it a little (2-3 mins total) during this time.

I am not incredibly flexible, but I do some yoga and I like to stay fairly active. What can I do to get back on the horse without this happening again?

Have you had your running form looked at? Are your shoes new or old? What kind of strengthening have you done for the area?

I am clipped in. Actually wondering if shoes were too tight. I loosened them a little today and had a lot less discomfort. Still feeling it but its changed from walking being excruciating to just a little annoyance after a few days off.

I have not had anyone look at my form. Its probably not what it once was. This would be in relatively new shoes (3-6 mos. Brooks - which I have worn for years), with good cushioning and stability.

I have not done much in the way of strengthening these muscles. I have done some weighted calf raises in the past and I do some weighted squats and lunges periodically, but nothing substantial.
I'm no Phil Mickelson, but my calves still have some decent size and strength for my age (43).

Thank you! I am going to work on those, seem easy enough.

Do you think its my soleus instead of the medial gastrocnemius? It definitely feels like it is part of or behind the medial head.

Hard to tell without seeing you in person but very few focus on strengthening the soles.

Minor adductor (maybe pectineus) pain at the origin with deep squats, possible minor strain. I don't recall any injury, but maybe I went a little aggressive on the stretching I've been doing lately.

Better to just stop all activity and let it heal all the way, or just back off on activities that aggravate it but still do them? Like squats, just not as deep? Or running, but no stretching of the area before/after?

Can't really palpate any trigger points, but worth trying to get needled?

I fractured my fifth metacarpal on my left hand on December 23. The orthopedist thinks that I may have got my fourth metacarpal on my left hand, too, but so little that it isn't easy to be sure.

It's not as sore now as it was even a couple of weeks ago but I don't have much range of motion in my left hand.

I can bend it down about 10 degrees and barely bend it up at all.

The orthopedist gave me some exercises, but trying them I'm hardly doing anything more than holding something in my hand because of the lack of movement.

Any suggestions?

Worth getting needled. Don't squat as deep. Jogging should be ok.

Tried less deep squats, but still felt it, so I laid off pretty much all lower body stuff, babied it, now starting to ease back into activity. Doesn't feel like it really went all the way away, scared to start squatting again too soon. So I started some machine stuff, and was on the sitting adductor machine, and felt a pinchy type pain/discomfort near where the pectineus origin is when I'm fully open on the adductor machine. I was doing light weights, and the adduction didn't hurt, just the static open position. But butterflies and other stretches don't seem to cause any discomfort.

So, question: Continue to lay off and baby it, or did I baby it too much and now that "pain" will work itself out as it stretches?

Update: adductors were super sore after the adductor machine on the right. But not painful. Second time using it there was minimal discomfort, have been doing it and gradually increasing weight. Light stretching reintroduced, and light squatting. Can still occasionally feel something that doesn't feel right, but I'm not going to push it anytime soon.

my left shoulder pain has gotten better as far as every day movements, but any pressing motion still hurts like a *****. can't do shoulder press or bench press or any exercises close to that which is killing my routine. can't seem to get it right even after lots of rest, hanging, etc.

My son, middle school age, recently hurt his hip while sprinting. He is athletic and plays multiple sports. We had X-rays taken and his PCP concluded that it is likely a muscle strain (hip flexor). After 4.5 weeks of rest and light exercise, he was feeling good and wanted to test out his speed. Immediately into his sprint, he got the sharp pain in front of his hip and had to quit. Should he just give it a few more weeks, should we go to a specialist, how will he know when it's back to 100%? He gradually built up to running and was running (but had not tested full sprint) without pain. Thanks for your thoughts.

Thanks for doing this. I lift four times a week. I lift heavy doing only bench press, shoulder press, pull-ups, dips, squats and dead lifts. Lately my elbows have been giving me problems. It started on the outside of the elbow and sometimes feels like I can't fully extend my arms. My right arm is worse. The pain seems to radiate through the forearm and recently has moved into my wrist and has affected my grip strength.

I suspect pull ups may have caused or contributed to this. I don't want to give up on the pull ups. Is there anything I can do?

Yes. A little tender to the touch around the joint. I really feel it when I extend my arm and it radiates into the forearm.

Thanks for the hip specialist advice! It wasn't a muscle strain after all. He just needs more rest and it's good to know what's going on.

Any tips/drills on changing from a forefoot strike to more midfoot? I've been more of a sprinter but as I've gotten older, have transitioned to more distance running. I find forefoot running leaves me with really tight calfs.

Try to heel strike and you will probably end up mid foot striking. Less forward lean will help if have a sprinting background. Put yourself on a camera if really want to make sure. Less of a heel counter your shoes will help with this too.

Seems like solid advice. A little lite on biomechanics and a bit dismissive of orthopedic conditions but overall seems good.

I was having shooting pain running into my groin, felt like I got kicked in the balls. Thought it might be an infection. My urologist checked everything and said it was very likely from my back.

I've had two L5-S1 surgeries and an inguinal hernia repair. He said it's pretty common for inflammation and nerve pressure along those sacral areas to cause pain in groin. He recommended I see a PT that handles a lot of pelvic floor cases.

Would this just be the normal exercises that I did post back surgery, focusing on core and "gripping down" when performing those (bridges, planks, etc.), or are there a whole different group of exercises? I'd rather just do some at home and incorporate them into my workouts.

L5-S1 rarely refers into the groin area. Based on your hernia issues I would be sure to rule out any involvement there first. Often a complaint of hernia issues is the sensation in the testicles.
