Ah, so they DID have safety issues. Been wondering all this time, glad this government document confirmed that.

this happens in so many accidents -- the safety equipment is there, but it's not working or turned on

I mean the jokes really do write themselves.

The masseuse reported Watson grinding before the explosion occurred.
