I think an Appalachian Emergency Room could be extended to movie length. Maybe have one character play it straight with incredulity.

The problem that almost all SNL skits run into is while it might be a funny premise or character, trying to extrapolate that into a 90 minute movie rather than a 4 min sketch takes a real premise and idea. And most of the ones in the 90s had already been beaten into the ground by going to the well too many times in SNL and already weren't funny.

Even the skits at 4 minutes seem too long more often than not.
