My Mother is on a Ventilator- Now What?

My mother is fully vaccinated but had a breakthrough case and she got so weak so quickly. After a couple of days home she went to the ER and after 3 days of being in the ER, she went on a ventilator. That was three days ago.

My biggest question is that I'm trying to understand from day to day what vitals to track. Talking to the Dr has been extremely hard but when I do talk to him I want to have all my questions ready. I'll also just take any general recommendations about anything since this is new to me and not something I have much experience with personally.

If anyone could help, I'll take it.

And if you have a second, my mom can use a prayer.

1. Try to find reason behind weakness that led to vent.
-1A time frame to be weaned off of vent
-1B- is she in prone position

2. What treatments have been done?

3. Are they working? Next plan of treatment?

4. Don't really have any insight on stable vital signs bc the medications used to keep patient sedated while on vent can effect body systems in ways I do not fully understand. Just ask the doc/nurse what they are versus what they should be.

5. Have a person with you when you call to just take notes and repeat back what is told to you for accuracy and understanding.

6. Ask about plan for when Mom comes home from hospital. What will she need as far as medical equipment? Research a good agency that provides experienced nursing staff with good equipment and support to set up visits by home health staff

I'm so sorry. So worried about this for my elderly mom as well.

Hopefully one of the doctors on here can help you.

That's bunk, and it's not at all helpful to this guy.

.My mother and father are over 80 and they both are smart enough to know that the vaccine is NOT a force field around them and that it's not a guarantee that it will keep them out of the hospital.

On the other hand they're also smart enough to know that it was specifically designed to help fight this specific virus and is working really well in doing so overall.

My mother has lung issues. If she gets it she feels sure she will die. She got the vaccine ASAP.

It is the best chance she really has to survive this thing. Same for my dad.

Please acknowledge that the narrative about vaccine effectiveness has changed since the vaccine first came out. If we would all just be honest, I think there would be more trust among people. We were told that it would prevent illness, then we were told it would prevent serious illness, then we were told it would prevent hospitalization and death. It just hasn't panned out like we'd hoped for many people, especially the elderly. It's okay to admit that. It's not worthless but it's not what we thought after the initial studies came out. Delta hasn't been kind to vaccine effectiveness. There is just no denying that.

And no, I'm not anti-vaxx. My family who didn't get COVID are all vaccinated.

I don't watch much tv or pay attention to much media. I just know that vaccines don't work like magic. They aren't a force field. They can't keep the germs away. All they can do (no matter the vaccine) is help you fight the infection. My parents knew that, my husband and I knew that. I thought everybody who ever went to a science class knew that (but clearly I'm wrong).

Particularly with an airborne virus there's only a question of how well your body will be able to fight it when you're exposed.

I am so sorry, OP.

The breakthrough cases that are severe are very scary, and I agree with red, cdc said very rare with vaccines. This isn't the time or place to argue such but people were told they were protected from serious disease at the very least.

But you do read here and you know what was said about the vaccine when it was introduced. It's frustrating that Delta has gotten around the vaccine in many cases. That's no reason to insult people who were more hopeful about the vaccine than you were. You can't blame people like my elderly mother who believed what she saw on the news. Again, if everyone tries to be as honest as possible, it will help with the mistrust of the medical establishment that people like my mom are feeling now.

Talk a lot of with the nurses. They typically only have 2 patients at a time in the ICU. While they are still very busy, they're not also managing 20+ other patients and potentially handling other issues elsewhere, consults etc. They should be able to give you objective data points, such as kidney function, how much ventilator support your mom is needed, etc. Generally speaking they're not supposed to give a diagnosis, or give predictions, but some of the older ones have seen a lot and have a lot of experience. They can help guide what you need to be asking the doctor.

My mom was on a vent for a day before going on ECMO for 57, so I got to be an "expert" at asking questions. The advice to speak with the nurse is spot on. We rarely got to see the doctor (they liked to round before visiting hours) but spent a lot of time with her nurses. I was fortunate that my wife is a PA and was able to speak with her care team as a peer. I don't think they kept things from us, but it was good to have someone who could get the non-dumbed down explanations. Things we usually asked about:

Any change in medications?
What are the vent settings?
Any noticeable changes in her blood gas panels?
How are her creatinine and BUN levels? (Kidney function)
What's her white blood cell count? (Secondary infection indicator)
Does she have any other critical lab values that I didn't ask about?

zero evidence to support ivermectin and or plasma infusion in inpts. That has been abandoned.

This is fairly accurate. The nurses are helpful but will not give any type of prediction. They do appear to have 2 patients so my mom is very well looked after.

She's no better as of yet but no worse either. Seeing her in the ICU is both the highlight and low point of my day each day.

I appreciate all the T&P and positive thoughts.

Is it an either/or choice for baricitinib and tociluzimab? I have a close friend who is on a ventilator and the medical providers are telling her family they can't give her anything else. When the family asked about baricitinib (before she was put on a ventilator), they told them they could no longer give her that because she had already been in the hospital a few days. I'm confused. How can it be helpful for patients on ventilators if it can't be given after the first few days in the hospital? Maybe the family misunderstood? Maybe it was actually that she'd already been given tocilizumab so they couldn't give her baricitinib, too? If she continues to be on the ventilator for an extended period of time, is there a possibility of trying a different medication later? Can enough time pass to try and treat the Covid pneumonia with medications again?

I would not let a loved one leave this world without getting at least a dose of ivermectin. Maybe it's useless, maybe it's not. But it's cheap and it's consistent with the Hippocratic Oath. Whether or not I think it works is pointless. But it is easy enough to try, and a terrible thing to second guess about.

Right now it's baricitinib. That's all we have. Toci gone. I am Rx ing metric ton of baricitinib. But sadly it will run out. Near 100% severe pt unvaccinated.

Make sure the kitchen sink is being tossed.

Monoclonal Drip, Ivermectin, Prednisone Steroid for lungs, Hydroxychloroquine, IV with all the vitamins jacked in it. Literally toss the damn sink at it. I personally took all of it and if my parent's life was on the line I'd be demanding all of it immediately. Good luck

Just a quick update. Her vitals are improved over yesterday and she is finally off 100% oxygen and at 90%. It's not a lot but it's something and I'll take it. She's still sedated and on the ventilator but at least she's breathing better.

I super appreciate all the positive thoughts and prayers for my mom.

I'm hopeful this trend continues but she still has a long way to go.
