I think it caught the underside of his cleat, which you might not feel much at all and wouldn't make you alter your stride.

I've done it before. It feels weird or off and often takes a second to recognize what happened since there isn't pain. That's a good explanation. It certainly wouldn't be as painful as the upper of the shoe, but he had to feel contact.
