Sewer clean out in driveway

So we're in the middle of a large driveway expansion. Well apparently there was a covered up sewer clean out that is right where we're expanding.

What's the best approach to handle this? Concrete guy is suggesting a a small enough box around it so that even if you drove over the box, it wouldn't be taking all the weight on the tire. I'm concerned that would t be strong enough though.

There's one in our driveway. It's got a brass cap on it. Never been a problem in any way.

Yup go to Moore Supply or any plumbing supply and get you a brass clean out

So once everything is framed, I'll need to install that so the top is flush with the top of the frame?

When you say "framed", do you mean "formed"?

If so then yes, when the edge forms on either side are set, pull a string across where the clean out is to give you the concrete elevation at that point. Make that the height for your clean out cap.

They'll probably pour over it and cover it up but you'll be able to tell where it's at. Just bust the 1/8" or whatever concrete is on top of it carefully.

I'm not a concrete guy or a plumber, just a stupid GC who's watched concrete guys do their thing for a while. If a concrete guy or plumber chimes in here with a different approach I'll gladly yield the way.

Usually see guys cover the brass lid with painters tape so if the concrete guys cover it, all it takes it busting the edges free and pulling tape off

Let them pour concrete. Cut the pipe off flush with the concrete and glue a flush plug inside the pipe. It comes with a chrome cover.

I'm not sure if Lowe's/HD carry these. You may have to go to plumbing supply house.

Ok ran into a minor hiccup. Green pipe running up from sewer line is I think 3034?pvc so it's OD is only like 4.25" while the clean out cover is for schedule 40 etc with a 4.5" OD.

I have to assume there is an adapter? Any help so I can find quickly?

So, what is the best practice or code for what's under the metal cover? Mine has an open cutout in the PVC drain line. I can see the affluent going downhill. Shouldn't this be capped with a PVC lid? Someone mentioned earlier of a green vertical PVC pipe as well. I have that too, and I can see all the way down, once I pull the metal cover. Lastly, they did a smoke test recently in the neighborhood of the sewar lines - I had smoke pumping out of several of these covers...
