That's not bad.
Set your expectations; you aren't getting 48sf of concrete for $4-$9/sf from a concrete sub. You might pay that in labor if you hire a guy from home depot parking lot and buy the concrete and form it yourself.
My neighborhood replaced a section of sidewalk recently, demo and install for ~126sf was $3K.
If you put in a 1000sf patio, then it may be $7/sf.
I work for a general and we install a million or so sf of concrete a year, and we pay, all in, $4.60-$4.80/sf for 6" slab, or about $4.40 for 6" city sidewalks, and that's with us getting the concrete at $115/cy.
If it's 4" unreinforced, we pay ~$4/sf.
Why replace it anyway just for tripping hazard? you might be better off grinding it.
