Good question, and an even better question is "Why in the hell is he telling Parsons and Beverley to switch right before the ball is inbounded?" Did they really not know who they were supposed to be guarding (after TWO timeouts)?
Here are some possibilities for that second question:
1. Players not paying attention (I'd find this hard to believe, given the circumstances).
2. Players being really stupid
3. Players panicking/freezing in the moment (this is much more likely than #2)
4. Players didn't fully understand their assignments due to the coach not being clear.
A teacher can know their subject matter really well and be a horrible communicator of that material to others.
Similarly, a really great player may (and often does) have trouble communicating strategy to their players when they try coaching.
I think McHale is pretty good at the interpersonal stuff and keeping players happy, which is not unimportant, but you've got to have some good X-O assistants to balance that, and the Rockets do not have that balance right now.
