Take your best guess, but what % of the time do QB's know what they're doing before the snap? I'd say 80%

Not if it's a true RPO. QB is making a read before deciding what to do based off alignment (sometimes) or how his key (DE generally on run option, or safety, cb, lb, or combination of those for run/pass or multiple pass options).

Now if by "before the play", you mean an alignment read that determines where he's going, then yes.

But most true options will be determined by how the defense plays after the snap, meaning 0% of the time he knows where he's going with the ball before the snap.

I can only speak for myself, but no, I never did.

That would be stupid and would take away the advantage of running an option.

This is the whole point of option. QB makes the decision during play depending on what the defensive read does. Otherwise its not option. Option adds complexity which causes problems for the defense, ie they are never right.
