I've had chronic sinus infections and allergies since I was a kid. I recently went to Dr. Kaplan in Houston, who I believe is one of the godfathers of this procedure, and a CT scan verified that I'm a good candidate.

Has anyone had a good experience with this procedure? He has the options of a local anesthetic or the twilight sedation in office. Any recommendations there?

I had one earlier this year to try to help with sleep apnea. It hasn't done anything for my sleep but I breathe much easier through my nose and no longer mouth breath.

The procedure was in-office and wasn't too bad. They just numbed me but I was completely conscious. I forget what the injection was but there was something that made my heart race and made me feel like I was having a panic attack. Very weird feeling since I've never had a panic attack.

After the procedure wasn't too much fun once the numbness wore off and it was pretty painful the rest of the day and my nose was bleeding as well.

I hope nobody told you it would help with sleep apnea (unless it was done in conjunction with other procedures).

Post images of your CT scans. Try to scroll through the "Coronal bone" images, ideally, and get images as close as you can to the following:

I had allergy shots as a kid. Highly allergic to pollens, mold, dust. My nose stays inflamed year around with the annual sinus infection. The ENT I saw proposed doing the balloon, taking a culture for mold testing, cleaning me out and then potentially starting back on allergy shots. I heard from another doctor that the balloon sinuplasty benefits might be short lived if the chronic inflammation cant be brought under control.

Which sinuses is the surgeon proposing to dilate? From just the report, the frontal sinuses would be good candidates for dilation.
You've already had sinus surgery, and your maxillary sinuses as well as at least the left ethmoid sinuses have already been opened. There should not be any need for any dilation of the maxillary sinuses. If your doctor wants to dilate your maxillary sinuses, you need an answer as to what, exactly, that would accomplish (hint: nothing, if the maxillary sinus surgery were done properly).
The sphenoid sinuses would probably respond to dilation as well.
Honestly, seeing the images would help a lot, but those are initial impressions from just reading the report. Also, it sounds as if your middle turbinates have been partially resected, of which I am definitely not a fan. To each his/her own.
Again, see if you could get your pics with slices to as closely match the ones I posted. Try to get the "Coronal bone" windows and scroll through to match.
