Looking for a good recommendation - decent chance I have to have surgery on my shoulder.

You have to stretch before using the swing.

Did a doc diagnose "shoulder" for sure?
My shoulder miseries were referred from cervical spine degenerative disks. Pick your poison! Took over 5 years to pin down the source with a lot of wasted cortisone. Just verify. Spinal nerve issues cause a lot of missed diagnoses.
If it's clearly ball/socket only, good luck. A tough one. Get a couple opinions.

Had a previous MRI and a few cordizone shots but it's gotten much worse where I'm in pain anytime I raise my arm or move it inward. This shoulder has been giving me issues for about 3 years.

Edit: just found my notes from the doctor from last MRI about a year ago:

Rotator cuff tendonitis
Small Slap tear - bicep tendonitis
Sub acromial bursitis
Cartridge is thinning out (little arthritis)

Can you tie this to a specific event/injury (Coryatt-type hit) or a repetitive motion issue (pitching, tennis, boxing)? If so, your symptoms kind of line up with a pure shoulder issue, given your input. There's a sports medicine dude, Dr. Braunreiter (Methodist-Sugarland) who I saw in 2016. I'd recommend he do a scan in case there are less aggressive options...If you're an ex-jock, you'll like this guy. I'm not a doc, but what you posted isn't definitive IMO. I simply know that major shoulder surgery....OK, but only after you eliminate everything else....if you find it's a spinal/nerve problem, that's often a more successful/less invasive solution. Methodist has a long list of good orthopedic guys, no problem. Just know this; neurosurgeons and many ortho guys prefer to cut. I'm cut; wish I'd gotten more opinions.

I'm sure hes a fine surgeon, but I'd recommend a specialized Orthopedist - hand /upper extremity vs foot/ankle vs knee.

Dr. Gerard Gabel and his practice group. I'm not a patient of his but am professionally acquainted.

I've worked in this industry since 2010. When it comes to shoulders I don't think it's a question: Marston Holt in Clear Lake is the best in Houston. I've sent him everyone I know since 2012. Several colleagues of mine in the industry feel the exact same way.

McCollough is a great surgeon. He did my sister's ACL repair at my recommendation. I think he's one of the best. But for shoulders, this guy can't be beat.

Trust me on this one

The only way I would recommend this is if you went with a shoulder specialist. No chance I would let a hand surgeon operate on my shoulder. Keep in mind that most (95% or more) orthopedic sports medicine arthroscopists work on knees and shoulders. Foot/ankle is an area where some will dabble, but most good f/a guys only do f/a...just like most good hand surgeons only do hand and some elbow (distal biceps rupture etc)

I work in the OR with orthopedic surgeons 5 days a week in the Med Center...including some that have been mentioned in this thread.

About 5 years ago I was diagnosed with a slap tear in my shoulder. Dr. John Seaburg with Houston Methodist did my surgery. I would recommend him if you are still looking for a doctor.

Dr. Timothy Noonan with UT Physicians repaired by posterior labrum tear 2 years ago. I have had no issues since completing physical therapy and I've been able to do strength training with no problems.

Carl Polumbo did my distal bicep repair and does nothing but hands, elbows and shoulders. The Orthopedic Sports Clinic is an incredible facility with great staff. Separately, I had C5-6 fusion from Neil Badlani out of the same practice.

Dr. Adickes at Baylor. As a fellow surgeon I can tell you he's the best.

No idea.

Grew up playing baseball until 8th grade and have played basketbal my whole life but can't see how that would have caused this...

I have bad knees/ankles etc...so who knows. I'm convinced my body hates me. 2 knee surgeries and knees are still terrrible, had a floral guided injection in my ankle last year, and 2 cortisone shots in my shoulder within the last year.

Hey Derek, can they do a guided steroid injection in that area, like your old ankle procedure? That would be a pain-mgmt. specialist, probably, if not an ortho guy. It may be too broad an area to do, but a Doc could use your results to help diagnose, PLUS, of course a real opportunity for pain relief. As you know, that's minimally invasive and a one-day turnaround. Just spitballin'. Probably had a half-dozen cortisone shots in my shoulder with meh results, btw. Over a dozen of the guided injections for referred shoulder pain from cervical spine...maybe 50% success in pain reduction. More important, inflammation reduction. Just don't know about your shoulder tendonitis and bursitis in re: guided injections. I'd check, though!

I just read my post and the targeted "relieve spine pain" ad that popped up next to it...
