Hand over heart has been in the Flag Code for a long time - since the 50s IIRC. I know it, along with stand/uncover, were taught while I was growing up in small town Texas.

I noticed it in the Correction portion at end of this article. Original said it wasn't in the code, but corrected to say was added in 2008.
