Medical Waiver vs questionable severe reaction

My wife has not sought out vaccination because she had a severe reaction to her first flu shot in 2012 that sent her to the ER where it's documented her heart stopped. She has asthma and fibromyalgia so we have mitigated her risk by reducing exposure - she has worked telephonically since fall '19 and I took over grocery shopping and other errands. We thought we'd outlast the populace and get through COVID without issue.
Yesterday, her employer (giant healthcare affiliate) hosted a meeting to field questions pertaining their implementation of a vaccination requirement among all employees, regardless of position.
Her PCP has told he probably doesn't think she should get the vaccine but has become shifty when pressed about getting a medical waiver. She's not morally against getting the vaccine (and would prefer to not make waves with her employer as she is moving into management) but given her history I'm not sure we could even find an office willing to administer the shot.
Is there a particular vaccine that we should seek out because of its track record among those with a history of severe reactions?

On a related note, why does the CDC not have a published medical waiver guideline? Browsing their website becomes an infinite loop back to "discuss with your PCP." Pretty unacceptable considering vaccination mandates are all looking for a specific card but a medical waiver seems be akin to a handwritten hall pass.

Update: We got the medical exemption from her PCP. It's a joke of a form that provides no room for further supporting evidence. She submitted it anyways but has already hear that even if it's not thrown out immediately, it will not be accepted in place of vaccination proof so her future at the company will be limited.

She has an appointment with her Rheumatologist who can hopefully provide some guidance on which vaccine is less likely to kill her.

The mRNA vaccines are very different from Flu vaccine and shouldn't trigger this terrible reaction. I would proceed with your plan to ask her Rheum though.
