Update from ER/Urgent Care

ER:
Shifts are unbelievably slow. We are sending staff home ( I was asked to sign out 2 hours early today) because of how dead the ER was.

Don't get me wrong....we can still have busy days, but overall, volumes are down. In my 8 hour shift today, I had zero COVID patients today ( I was one of 4 providers working during that time....can't speak for them). The tent outside the ER in the parking lot sits empty and waiting.... (better to be prepared I suppose).

We have been admitting 1-2 COVID19 patients a shift. This is a very manageable number and actually less than what we would normally send to the ICU on a normal day. With the decreased volumes....it is all very manageable

We are still waiting for the "peak" which we are told is going to be in about 12 days. If that happens, I feel like we are prepared. Over the last 15 years, I can not tell you how many shifts I have worked with all 36 beds in the ER full, 40+ patients in the waiting room, all beds in the hospital full, and boarding ICU patients in the ER. I am not trying to downplay the pandemic by any means......but this is a currently a walk in the park compared to an average shift.

Opinion: I feel like we have done a great job as a community to limit the casualties. Social distancing and community education has worked. I think it would be a much different story if we had not locked down. The elderly are very susceptible to this virus, and we have done an incredible job at keeping them safe. Community spread in the young and middle aged population has been widespread, but not devastatingly lethal. In addition, folks are staying home and not coming to the ER for BS. The normal non emergent complaints I would see in the ER have almost disappeared, probably due to "fear of the Corona Virus".


Urgent Care:
The Urgent Care I work for has tested over 2300 patients in the past 2 weeks. They are offering self pay testing, so no criteria needs to be met. If you want the test, and have $120, you can get it. Turn around for those tests have been on average 2.5 days. We are right around 17% positives of all folks tested. I have lost count of the number of asymptomatic people that I have scheduled for testing.....and many others have symptoms, or had known exposures.

Our volumes are way down. About 50% of what they used to be. People are just staying home.

They are cutting hours there as well.

We see all patients virtually if they have URI symptoms or concern for COVID19. Otherwise, we will see you in the clinic.


As stated in my previous post, Im a PA who has worked in emergency medicine for 15 years and just started picking up at an urgent care recently. I work in the Houston area at one of the large system's community hospitals.

I am an EMT and have had several patients ask where can I get tested. I refer them to the covid-19 info hotline. Do you know if all urgent care facilities are testing or just the facility you work at?

Our ER folks are telling us very similar things. Basically the only people coming to the ER are the people who are really sick. The people who are coming for random/stupid crap aren't showing up. Same for the people with moderate symptoms who may or may not need hospital care, and this is not necessarily a good thing for them. I'm worried about the people who are toughing out chest pain at home because they don't want to go to the hospital.

We are seeing a lot of hepatic encephalopathy / ascites cases over the last couple weeks. Kind of a strange thing.

Our medical inpatient floor is the most empty I've ever seen. However, the progressive care floors are pretty full and ICU is sitting slightly above average but we have beds. So basically, only the really sick seem to be coming to the hospital.

Our grandson will graduate from college next month (pre-med major). For over a year he has worked night and weekend shifts as a Scribe at an ER in an IL suburb of St Louis. He used to work 6 hour shifts, but after his university shut down several weeks ago they put him on 7pm-7am shifits.

When he was about to finish his shift this morning the company (ScribeAmerica) that has the contract to supply Scribes to his hospital told their Scribes who work at hospitals all over the nation that they were now considered "non-essential" and were being furloughed..

The Drs he has worked with for over a year are really pissed about it, but apparently there is nothing they can do about it. They really depend on the Scribes to take notes for them during their ER procedures, compile drafts, and then complete the final versions for the Dr's patient records. Not having Scribes available increases their workload a lot.

He lives/eats meals at his parents house and has been very frugal with his earnings, so losing a paycheck is not going to be a huge impact. He can use the time at home to study for the MCAT test that Medical Schools require for their application process. But I suspect that may not be the case for a lot of his fellow Scribes around the country and many will be hurting financially.

From reading a lot of posts on this board the response to the Coronavirus is playing havoc with normal hospital operations, etc.

An urgent care near me has antibody testing for $35. That seems reasonable enough that everyone should attempt to do it (obv. I realize logistically this is challenging) but it's good to know cost shouldn't significantly stand in the way of gaining more knowledge on the spread and past spread of this virus.

We were just talking about this the other day.

In my opinion, three things happened (with the understanding that the ER was never the main source of the drug seekers "supply"......the ER was the place they came as a last resort when their normal supplier failed them)

1. Triplicate forms for Norco. This has made it much more inconvenient to prescribe Norco and it has given providers a reason to say "no" (either because they don't actually have the prescription pads or they're being much more selective because the pads are expensive and "limited").

2. We stopped carrying dilaudid in the med dispenser located in the ER department. It made it easy to say "we don't have dilaudid". It's amazing how the patients requesting "the one that starts with D" stopped showing up once they heard we don't carry it anymore. Of course we carry it and would never withhold it if medically necessary

3. Corona has folks staying home!
