I haven't done a ton of research, but I'm liking Vanguards 529 plan. Mainly because of the low expense ratio. Anyone have a plan they particularly like or don't like?

For my oldest we did Nevada (Vanguard funds), but the younger kids we did New York because I think their expenses on their Vanguard funds were lower. I want to say most places had .25% or so for their expenses, and New York's was .17% and they were all the same basic Vanguard funds.

We are using Fidelity New Hampshire. We use the Fidelity 2% Cash Back 529 AMEX for our business purchases and over 12 months have accrued more from our small business expenses on the credit card than anything. I strongly recommend looking into the 529 Amex

Just applied for this card today. I already have a Fidelity account, so do I go ahead and create a 529 online and then just link the card to it once I have it?

That's mainly why I chose Utah along with its high ranking, which I suppose go hand-in-hand.

Unknown. My financial adviser recommended it, even though he does not sell it. The fact that he pushed me toward it over products on which he could have earned a commission was profoundly motivating for me.
Utah's low fees range from 0.18% to 0.26%, and for the customized options from 0.20% to 0.59%. There is extra benefits for Utah residents and those who support BYU football.

Yea, looks like that New York plan beats them all. Same Vanguard funds with the lowest fees.
Heavy rollover penalty on the NY plan. Doesn't have to be a problem, but should be aware in the event that you might ever need/want to change the plan you are in.
