I have been on PPIs for 8-10 years for GERD. In 2016 I had an endoscopy done previously and saw erosion of my esophagus and slight hiatal hernia. Shortly after my doc retired, and I have just been getting refills of my Dexilant from my GP in Houston. Had a gastro in Houston for a brief dime but didn't really seem to offer much help. I have since moved to B/CS and wanted to get something here.

I cut out alcohol 100% and that has helped, but would like a drink every now and then, without the threat of reflux.

If I do not take Dexilant every day, symptoms are swelling throat, difficulty swallowing, acid burn in my throat.

Any recommendations for a Gastro that will actually help to get to causes of this? I live in Bryan, so prefer in BCS, but if there is no one here that is decent I would travel. Tired of relying on medicine.

I preface all this by saying that I am not a GI doctor, nor a stomach surgeon.

You have sort of answered your own question. You have a hiatal hernia, which means that stomach contents, acidic or not, can come up out of the stomach and burn your esophagus. The reason to NOT ignore this is that it can be a precursor to eso****eal cancer. The stomach has a protective mechanism to tolerate extremely low pHs, while the esophagus (and tissues higher up) do not.

Dexilant only decreases the acidity of your stomach contents, it does nothing to change whether or not you reflux. I would try alkaline water and adherence to the Mediterranean diet as well as you can. Additionally, you might be served well by trying alginate therapy - taking Gaviscon Advance (ordered overseas) after every meal and at bedtime. It forms a "seal" or a physical barrier to keep stomach contents down. Also, let gravity be your friend as much as you can, even though it's possible to reflux upright as well. Make your last meal, especially, as early, light, nonfatty, nnonspicy, and easily digestible as you can. If you need to, sleep on a wedge or whatever you need to do to keep your head higher than your stomach. You obviously have come to discover the effects alcohol has on your stomach. If your hiatal hernia is severe and you are uncontrolled otherwise, there are surgical options for taking care of that.

Thanks for the response.

I have done a few of these things by changing diet and Gaviscon is a god send when I need extra help. Dexilant keeps it manageable 99% of the time. But I do not want to be reliant on it forever.

I understand hernia repair is probably in my future.

Not sure where you are located, but any doc recommendations in BCS would be greatly appreciated.

Gaviscon (and get the Advance formulation, as it is more concentrated) should be tried on an ongoing basis, not as an as-needed. I live and practice far, far away. It really shouldn't be too difficult to find someone there to help manage this. The most important thing is to make sure you aren't having eso****itis or Barrett's eso****itis that isn't being followed.

Earlier this year, I underwent a colonoscopy and an endoscopy and was diagnosed with Barretts esophagus (BE). I suffered from GERD for many years which resulted in the BE and know what causes the big flareups in GERD. Sweets, caffeine, alcohol, fried foods, and spicy foods are the main culprits! Being overweight is one of the biggest contributors to GERD but I'm on a diet and exercise schedule to drop a bunch of lbs this Summer and the rest of the year.

I lost my oldest brother to eso****eal cancer last year and he previously had all of the GERD symptoms that I have. It was an eye opener for me which prompted me to schedule the endoscopy. I'll be 61 years old this year and started making a lot of life altering habit changes to live a longer, productive, and happier life.

Have you ever tried mastic gum ?

This has been a game changer for me after fighting gastritis and stomach acid issues for a long time.

No i have not. Dexilant has it pretty much under control for now. But hopefully can get to the point I do not need it anymore. I will have to try it.

I have no great beat guy ever recommendation but family has used chris derbes at central texas gastroenterology. Wife has seen pham there many years ago, daughter seen moore, everyone likes kacey keeshnic PA. But it seems gastro is one. Of the many hidden causes and never the answer you want.
But everyone there has been competent and better than other places in the area

I'll just say, I had GERD pretty badly in my mid-20s. Losing weight basically solved it 100% for me. I wasn't that heavy either, was 6', 215#. Lost down to about 190 and 90% of my symptoms went away.

I wish that was my problem. Would be a quick fix. Weight is not my problem. For back ground I am doing 2 Ironmans and an 100 mile ultramarathon this year.

I see dr derbes at central Texas gastro . I like him a lot and has stayed on top of my issues . Sugar , caffeine, alcohol (in particularly , wine) all make the acid worse . I also had a hiatal hernia , but no one knew , until last year , that 75% of my stomach was sliding up into my chest at times . No wonder my heart hurt . . Anyway , had the surgery to correct hernia and have done much better . No meds except an occasional Tums .
