Congested/stuffed up nose

Damn near 2am and haven't slept for a minute. Completely stuffed up and can't breath through my nose.

What is the best OTC medicine or thing I can do to fix this? I've been taking sinus medicine, Tylenol cold and flu and neither have helped.

This sucks.

Sudafed. But make sure it's the one with pseudoephedrine in it, not phenylephrine.

Can also use afrin for no more than 3 days. And can combine with a netipot.

Afrin or the generic nose spray. I'll only use it before bed but that's the only guaranteed fix when I can't breathe. Neti pot can only work if you aren't completely congested. Super spicy food does work too but that's not really a bed time remedy.

I recommend a nasal spray called Xlear.

The nasal sprays can work pretty fast. The real sudafed also is very effective for several hours.

Short term: Afrin and sudafed.

Long term: Daily shot of Flonase up the nose. Haven't had a sinus infection since I starting doing this.

I used to get sinus infections and stuffy noses all the time. I started using Flonase daily for vertigo actually and ever since then (3 years ago) I haven't had one since

I'm a big fan of Azelastine HCL (rx) nasal spray. Works for allergic and non allergic rhinitis and can be used as needed.

That's a lot of steroids daily for 3 years

Try antihistamine like kid doc recommended above

You don't have to do it daily for it to work like steroid. Just use it when u need it

When I changed allergists, the new one suggested I try this instead of going back on shots. Other than the taste, it's done great and I can still take a zyrtec if I do something that really hits the allergies.

Looks like it's going non-prescription, does it provide prompt relief or does it need to be used regularly?

My sinuses get congested within a day or two if I don't use a sinus rinse/nasal spray.

It's an antihistamine so it provides fast relief...it does taste bad though

The taste is a non issue if you don't snort it, just rub it in. Also you can put a little bit of Splenda in it.

It should be OTC, very safe and effective. Works within 10 minutes.

Doesn't appear to be OTC as of yet that I can find, best I've see is that Bayer got approval this past summer with expected availability at the beginning of next year.

my allergy dr. prescribed me Dymista which is both Azelastine and Fluticasone. Changed my life. I would get severe allergy attacks that wouldn't let me sleep for days until I started Dymista. I now take it nightly (sometimes in the morning as well) and pop a Zyertec when needed.

Glad it works well for you!

I would consider changing to Fexofenadine (allegra) for as needed oral. Zyrtec can make you a bit drowsy or spacy.

I keep up an old school regimen of Alkolol mixed 1/4 or 5 parts water

Takes a few days to clear out funk but then once a day does the trick

If I every have an attack I use WalMart generic nose spray (like Afrin but different ingredient) but alternate nostrils, only one per day, seems to keep things from "locking up" where nose spray becomes ineffective after too much use

We have one of these as well, if you're really congested and don't want to take nose spray. You fill it with water and it has menthol tabs you put inside so you're breathing a combination of steam and menthol.

Afrin is OK for about 3 days or so but then you start to "rebound" and end up more congested than when you started. There is evidence that if you use Flonase or other nasal steroids daily, there is some degree of protection from that rebound. The dosage of Flonase systemically absorbed is nearly negligible, particularly if used properly. A researcher once did the math on it and it ended up that daily use of Flonase for 7 years was about the same amount of systemic absorption as a Medrol dose pack. Only if you have glaucoma should you really worry about it, IMHO, but all steroids contain that same class warning.

The systemic uptake of Flonase and other nasal steroids is significantly increased if proper technique isn't followed - look down at your toes, gently breathe in (sniffing hard just means you swallow the product instead of getting it to where it needs to go), and aim slightly to the outside (about as much angulation as your fingers would create if you made a "peace" sign over your nose). I particularly like Flonase Sensimist because it has low systemic uptake and a "stickier" formulation without alcohol and the nasty fragrant taste that the prescription Flonase has.

Azelastine is great, but it tastes nasty. I use it a ton, and it is apparently going OTC very soon. Although it tastes nasty, but you can add Splenda to it, and it takes the bitter taste away (Kid Doc, how did you know that???). Another alternative is to use Astepro instead of Astelin (50% higher concentration, dosed only at night, and contains Sucralose and sorbitol to take the nasty taste out). Astelin plus Splenda (added to taste) is the poor man's Astepro.

Oral decongestants work, but if you have hypertension or any cardiac issues as well, be careful with them vs. just avoid them. Also, you have to sign them out from behind the counter if you want the good stuff because people make meth from them. One of my colleagues was doing this years ago, and doing it very well. He ended up going to the Big House. I had a patient who was DEA who was working the case, he told me he was making a lot of it and making it quite well. Perhaps he was the original Heisenberg from Breaking Bad???

Oral steroids are the final arrow in the proverbial quiver, but they have lots of risks and so you need to weigh that against the potential benefits.

You mean rub it inside your nose instead of squirting it up there? Did not know this was an option.

Take the top off of the Astelin bottle, put a bit of Splenda in it. It doesn't take much. Shake, spray. Add a bit more if needed.
