Ran a ragnar last November, which is not unusual for me, and have had pain above my right butt cheek ever since, just below the iliac crest. The pain ranges from a dull soarness to a sharp sudden tingle radiating down my leg depending on my activity. It has regressed so that i only notice stiffness and pain after intense physical activity. I can get about a mile and a half run in now before I have to stop. I also notice it going downhill specifically on long hikes. It has improved but progress is extremely slow. I even went to PT, which ended up being a waist of time since they didn't find anything obviously wrong. Any thoughts? TIA

If it's radiating down your leg, my best guess would be something is irritating your sciatic nerve. I'd assume your PT probably thought of this, but if not, youtube "piriformis stretch" and "pigeon stretch" and do those. And also take a tennis ball, put it on the ground, and roll the affected cheek over the ball. You may have to cross your leg to get good pressure. It'll probably be painful. Start with the stretching first.

Check your IT band as well. I would recommend a good foam roller.

I tend to have problems with that area and have found that it runs all the way from my lower back to my knees. The recommended stretches above help and the foam roller seems to help as well. For me, it is a constant battle.

+1 on sciatica/sciatic nerve pain.

Foam rollers will help, as well as deep tissue/sports massage.

I have also found that using a baseball or softball can be great for pin-pointing muscle knots in the piriformis.

Did the PT look at your SI Joint to see if it has slipped? If your SI has moved it can trigger all the S/S that were posted above and they usually only act up when you are active on it.

Sounds like a herniated disc with sciatic or radiating pain. The stiffness you feel after cooling off is a residual effect from the disc expanding after being heated from running. The PT being a waste is hard to determine from your comment. Was it a waste because you were not 100% better or because you can't return to your previous activity level without pain? Or was it the PT didn't educate you or didn't know how to treat you? Anyway hope you Re feeling better. If you do have a disc problem running will make it worse.
