Depends on size, how big the barb is, where it has to go to get pushed through, if the barb is exposed or buried, and if you can cut the eye off, etc. Each situation is a bit different.

I once saw a guy remove it with fishing line on a fly fishing video. It was pretty amazing and painless. I'm going to the hospital if it happens to me.

I cut a hook out of a HS kid's hand on Matagorda last summer. Somehow he'd gotten a big, fat treble hook through the meaty part opposite the thumb.

I had to cut away the main part of the treble hook, pull through the remainder, and send him to the hospital for a tetanus shot.

This. I buried a hook from a topwater in the back of my hand, and the other hook had a skipjack still on it. Luckily, there aren't as many nerves in the back of your hand, so the pain was minimal but it did work.

Multiple ways depending
1) push it through - many times you don't want to do this
2) thread an 18 gauge needle down shank to barb, pull out
3) apply do ward pressure to the eye (effectively pressuring the hook down away from the path of the barb) and yank with a sting attached or around the curve of hook.

I like the third way of possible, pictures make it much easier to illustrate. But I've just yanked them out with pliers on myself

Some of this depends on where and how deep it is hooked. The couple times I have done it I just ripped it out. If it was only under the skin slightly I may try to push it through but i never have done it that way.

I think the fishing line trick may work but sounds dicey in a real world application as seen on video.

Do you mean the meaty part below the thumb or the meaty part below the little finger?

You have to be very careful in the meaty part of the thumb because the recurrent branch of the median nerve runs very shallow there. That's not a nerve that you want to mess with as the thumb will no longer function properly if it is injured.

This is what I've done. Not sure it was the best, but it worked.
I was playing fetch with my dog, throwing a ball into a lake. When he retrieved it, he was all tangled up in fishing line. I pulled it off him not seeing the hook, then tossed the ball down the beach again. He took off running and I hadn't got all the line off him and he set the hook into my hand. Luckily once it happened I called for him and for the first time in his life he actually listened, stopped, and turned around.
