I've got an older desktop. Specs are the following:
Processor: Intel(R) Core(TM) i5-3350P CPU @ 3.10GHz 3.10 GHz
RAM: 24GB
video: AMD Radeon 7500 series
HDD: st31000524as HDD
Windows 10

speed test:
upload: 151.4 Mbps
download: 249.6 Mbps

Sartup programs:
acrobat collaboration - enabled
adobe GE invoker - enabled
adobe updater - enabled
amazon music helper - disabled
amazon photos - disabled
apple push - enabled
catalyst control center - enabled
ccleaner - enabled
ccxprocess - enabled
citrix connection center - enabled

creative cloud desktop - enabled
delayed launcher - enabled
eset - enabled
garmin express - disabled
chrome - enabled
google drive - enabled
google update core - disabled
HD audio background process - enabled
iCloud services - enabled

makes sure spybot 2 is there on Windows 10 - enabled

I find that it is very slow to start up and frequently get a "not responding" notification when doing work (medical software - EPIC via citrix gateway).
should I disable some of the software?
should I just get a new system?
should I do both?
any other suggestions

Super fast. Just buy a new computer. I did this for photo editing and the difference was astounding.

I agree with that and just bite the bullet.

AMD R9 7900x cpu for about $400. If you bump it up to around $600 I believe the 13900k and 7950x are both great choices.

The only upgrades I realistically see for that one is upgrading the hard drive to a solid state and possibly upgrading the CPU depending on what motherboard you have.

It would require a fresh install of windows so it might not be with the effort. I think even stepping up to an entry level modern computer would be a massive upgrade so I wouldn't throw good money after bad.

I don't think you need to go as high end as the others suggested an i5 with integrated graphics would probably be just fine for your use.

A $50 ssd would likely fix the boot speed and not even an i1000000000 will make citrix any faster. The not responding is also likely from citrix experiencing network issues and again not going to be fixed by fancy new computer.
Fancy new computer is always nice, but i would try to find someone with the citrix client on there laptop that could try it on your network.
Some pictures of some of task manager and maybe some perfmon runs might find some Contention. But nothing you included indicates any need for maxiblasto cpu or video card.
Ssd and lots of ram cover a multitude of transgressions, and while spending money on old system may not make much sense i would do the ssd unless further evaluation shows some undisclosed need.

New SSD or M.2 if your mobo supports. Reinstall windows 10 fresh on new drive and reinstall your programs (onto new drive) and move files over manually. Turn old drive into file storage for not frequently used files.

OP, your hard drive is a "spinning disk" - mechanical in nature, and so as **** compared to new hard drives ("SSDs") that have been out for the last 10 years or so. Replace said mechanical drive with an SSD and you'll be a lot happier.

Also, Citrix is trash.

I literally have everything disabled on startup. I didn't scrutinize your list completely but almost nothing needs to start on boot. I'd rather start something I know I want than have it always on.

I also run absolutely no AV or spyware software on my machine. I do scans periodically but it is a download, then scan, then remove type thing. Safe browsing and a firewall pretty much eliminates that need.

This is a solid recommendation. The first step to provide more insight into the cause of slow downs is to open up task manager / resource monitor via Ctrl+Alt+Delete when you notice the issue. This can show you which application is bogging things down.
If this isn't a domain networked computer then I would disable all startup applications and to go a step further, set almost all services Windows will allow to Manual.

Yeah, I've never allowed any of my computers more than 3-5 programs to start on startup. The amount you have is very excessive, imo. New computers are nice, but yours is still adequate, so I'd suggest disabling the vast majority of your startup programs as a solution first.
