It was very close. The commanding officer of the Soviet detection system didn't trust the alarm. It was a new system that detected an ICBM, he didn't fully trust it, and he believed if the US were to attack first it would be hundreds of missiles, not one. They also had no corroboration from radar. Later, they detected 4 launches, and he had even less trust for the early detection system then and took no action. He was a phone call away from causing the Soviets to respond with everything they had. No. In September of 1983, we were pretty close to all out nuclear war with the Soviets. At first you had KAL007 and then later in the month, they thought we'd launched on them. How close is debatable but I think nuclear weapon use was indeed "on the table" even though it wasn't publicly stated or even known by more than a few at the time.
