Allocating limited resources in ongoing COVID-19 cases.....

I am struggling with this daily. Seems as if every new COVID-19 patient now is very elderly, nursing home bound, frequently with dementia and other major chronic illnesses. Many with DNR orders. They get so sick. 3 new ones today that fit these criteria.

The limited resources at my institution are drugs. Not ventilators. Ok on PPE. We have a few doses of remdisivir and tociluzimab at all times. I have been reticent to pull the trigger on these drugs for some of these folks. As, occasionally we have younger less co-morbid cases.

Trying times. Not to mention the med-legal risk involved. Curious as to others experiences and approach.

Just out of curiosity when you say you are fine on PPE, what does that mean? A new mask every shift? A freshly disinfected mask every shift? A rotating stock of masks that staff rotated through over the course of a week?

Our policy is a N-95 is good for 8 hrs. Then replace. We have been a little scarce with small size N-95s (as most RNs females with small faces.) Ok on regular size. Also full stock of face-shields.

Just curious. Have you utilized convalescent plasma for these patients? I don't have hard data but it seems most of the cases in our area are older and transfers from other counties that had outbreaks in nursing facilities. It seems every day is quiet until around 4pm, then from 4-8pm each night we get 3-6 orders for cvpls all back to back.

Yes. Rxd innumerable units of FFP. But scarce quantities.

It seems to be getting more scarce by the day. Regionally we are steady but dfw area seems to be using more each day, and more and more smaller hospitals are coming on board due to not being allocated other pharmaceuticals...

Assuming the patients are in ICU given your specialty.

A couple observations.
There is currently no approved therapeutic therapy for Covid 19.
The use of any drug or plasma treatment is off label and/or compassionate use as a means to treat the infection.
The default treatment of supportive care is appropriate.

The patients who are DNR have previously identified that they do not want want intubation or resuscitation.

Given the above observations, it is clear that you will not be able to obtain consent to treat with off label or compassionate therapy from the demented patients. It is unclear how likely any medical power of attorney would approve this either. Fortunately, supportive therapy remains an appropriate Standard of Care option for all patients.

Hopefully you guys will have greater number of therapeutics soon, although you will need patients to be able to consent to them. It will be nice when there are actual approved therapeutics and guidelines for all docs to follow.
