Hit post too quick -

How tough (expensive) would it be to add can lights to the 1st floor ceiling in a 2 story home? Difference for rooms with existing cans and rooms with no existing cans?

Depends on if it's an area you have attic access above and which way the joists run. If there's no access and you're going across them, a lot more "fun" than if it's the other way around.

Doubt it is too expensive in the grand scheme of things, more of a PITA due to patching drywall.

You may be able to minimize that a bit if you design around your joists but are going to need a hole cut on both sides of every joist/block to get the romex through to the other side.

W existing cans you can piggyback off of them but you still have to run romex, maybe just less overall.

Probably a 1 day job for a room (minus the patching/spraying/painting the drywall)

alot of electricians out there who advertise by the hour vs bidding on a job.. might help a bit..

You could always try your hand with a bit extender to span the space between joists. I realize this can be a difficult/tricky/frustrating task, but it beats patching drywall in between all your joists.
