I received a tax statement from Fidelity. It itemizes $1,200 in ordinary dividends from several stocks held in book form.

Should I put "Fidelity" on Turbotax schedule B or itemize the individual stocks?

Yes, i have procrastinated and the last day to efile is tomorrow.

There's no need to itemize individual dividends. Just list the ordinary and qualified amounts. Also if you have any sales as long as they're "covered" (basis reported to IRS), you can also list lump proceeds and basis amounts.

What the hunter said. In this case, the payer on the 1099 is likely National Financial Services LLC. That is the company name that shows up on 1099s for Fidelity. If you don't have any other 1099s, having Fidelity listed as the payer will work.
