1. The TV Provider determines what licensing your media package provides like ESPN, ESPN2, SECN, SECN+, LHN. A TV Provider would be typically cable, satellite, or streaming solutions like YouTube TV, Fubo, or Hulu Live TV.
2. Your ESPN+ subscription is entirely separate and only available directly via ESPN or more usually the Hulu/Disney+/ESPN+ bundle.
The term simulcast means you COULD see two or more different options for which stream to use and you want to choose one that matches licensing you have via either your TV Provider or your ESPN+ subscription.
