I had a ring for my wife custom designed at Montelongo's . They have a couple of designers on staff and high end 3d modeling software. Expensive, but worth it for custom work.

Chuck custom made my wife's engagement ring/wedding guard 10 years ago this October/November. Best purchase I ever made! He does great work, and he is quite reasonable!
