DVM Question - Dog Neutering & Swelling

My dog, Archie, was neutered on Monday. Everything was going pretty well until last night. We gave him a dose of carprofen before bed and when I woke up this morning he had vomited and had diarrhea in his kennel. I figured this was a reaction to the pain meds. In the night he managed to get his cone off and was therefore able to lick the incision, but to what extent he did I do not know. Took him back to the vet this morning to inspect the incision and they gave him an anti-nausea, an antibiotic, and a probiotic, and recommended a cold compress, but said things look pretty good/normal. They also recommended we stop the pain medication since he was a full day+ post op and it was upsetting his stomach. He was up and moving this morning and was described as "bouncy" at the vet.

Since then he has not eaten and hardly moved and the swelling has increased. He was laying on his side with one leg up for a while indicating some discomfort in my opinion, so I gave him a half dose of the carprofen with a little food. He still isn't eating other than his favorite treats, won't touch his regular food.

How much swelling is normal? His scrotum and ***** area are significantly swollen but I'm not sure how much is normal. Is there anything else I should do? Take him back to the vet (it's 24/7)? I'm hoping it is just pain and the dose of pain medication will have him up and moving but am getting a bit worried.

Edit to add he's a 50 lb Malinois/pyranese/shepherd mix and 6.5 months old

Update: made him some dinner of rice and wet food and he is eating it. Wondering if the probiotic (a powder you sprinkle on the food) smelled/tasted bad to him earlier. Also drinking some water. He's still not moving much though.

Sort of a weird pic but figured some sort of size reference is a good idea

Swelling is normal but it's a relative term. So is the term bouncy. They're more likely to bust a stitch if they're jumping around.

Edit: didn't see the pic when I posted this. Scrotum looks normal to me but I've never really seen the ***** area swell like that. If the vet thinks it's ok then I'd trust him/her.

It was not that swollen this morning.

Uh that scrotum is anything but normal and that's why he not feeling good today. Problem may have arisen as a surgical complication or could have also arisen from him traumatizing the incision by licking and being bouncy. I think you should take him back in the morning for reevaluation.

Not a vet. With neutering of any animal the area will swell large enough to make you say "dayum". Yes it will be uncomfortable(For both of you) for a while. Follow your vets advice. Ask when it is okay for you to take the dog for some short walks.

I guess I'm just filing that under normal because it happens a lot, especially with "bouncy" dogs.

Went ahead to the vet and he is also running a low grade fever (103.7) so going to do an antibiotic and pain management injection and change antibiotics and go from there. They did say they did an ultrasound of the area and saw some pockets of fluid in the scrotum but just inflamed tissue around the ***** area. All in all we are probably overly concerned about something that isn't too big of a deal but I hate seeing my boy in so much discomfort.

The tape measure doesn't lie........ he won't be a porn star.

Sorry he's in pain but I'm sure he'll be just fine even with some complications.

Swelling has continued and the incision is now leaking some bloody puss so we are back at the vet. He's so uncomfortable he can't sit still.

Damn, sounds like time to do incision and drainage, and culture the bacteria if they haven't already, and start on some IV antibiotics.

That's the plan it sounds like. Glad to know we are on the right path.

Good call, scrotal hematomas are more common in large dogs post neuter. I've seem some that have required a total scrotal ablation. In my experience its rate for them to get infected. He should do well with meds.

Well the issue all started when he was able to get out of his cone one night in a panic when he got sick in his kennel.

Just heard back and the infection is continuing to worsen and drain puss and blood, even with IV antibiotics. The incision is no longer closed. They are going to take him into surgery today to drain and remove infected tissue then decide if they can close the wound or if they need to leave it open for it to heal a bit before they can go back and then close it. Does this sound reasonable?
