Always water, never ice. Whisky Stones if you want it chilled. The problem with ice is that its not consistent. One or two cubes of ice can vary depending on where you are. you can always measure water out exactly.

I always order neat, with a glass of ice on the side. That way I can add as much or a little ice as I like, without being rushed to drink it, or the whiskey getting all watery.

If I'm at home: two medium/large ice cubes.
If I'm out: neat. The overwhelming majority of bars have crappy (i.e. soft drink) ice which melts WAY too fast. I would rather have my whiskey stronger than weaker.
